Complete and sign your full contractor packet through our secure DocHub portal. Includes:

IRS Form W-9 — taxpayer identification and certification
Background Check Authorization
Independent Contractor Agreement — covers scope of services, compensation, OSHA compliance, job site safety, and non-solicitation
Direct Deposit Authorization — optional, complete if you'd like electronic payments
